The NHS Health Check is for adults in England between the ages of 40 and 74. Chevin Medical Practice is keen to promote healthy choices and lifestyles and are offering all patients between the ages of 40 and 74 a free check of their lifestyle against known risk factors.
You will be asked a series of simple questions about your lifestyle, family history and health during your check and have a blood test taken (if one hasnt been done recently).
For those patients who are identified as at higher risk a second follow up appointment will be arranged to discuss the findings with a GP.
If you are interested in having a check, please contact a reception colleague to book an appointment stating clearly that it is a NHS Health Check you require as we have specially trained staff and need to allocate appropriate time to this process.
