We are mindful of carers' circumstances when arranging appointments. Please let us know if you are a carer or have any special requirements and we will do our best to assist you.

If you need to register yourself as a carer with the surgery please ask reception for one of these yellow cards to complete. This will register you with Carers Leeds who can provide you with information, advice and support to help you. We will update your medical record to show that you are a carer.

Carers Leeds Emergency Card

This card is free of charge and is designed to be kept with a carer at all times, for example in a purse, bag, wallet, phone case or on a key ring. In an emergency carers can show someone the card so that they will know that someone else relies on them. The card will include contact details of someone who has agreed to step in and take over caring for the cared for person for a period of time. This will usually be family or friends.

Carers can request a Carers Leeds Emergency Card by completing this online form.

As well as receiving the Carers Leeds Emergency Card they will also be added to the Carers Leeds mailing list and will receive a regular Newsletter by email or post. More information on the Carers Leeds Emergency Card is available on this poster.

Many things these days rely on technology – from online banking to appointments, and this can often be overwhelming at times. Our Digital Inclusion Project at Carers Leeds provides support on a one-to-one and group basis for carers who would like an introduction to digital technology or would like support in becoming more confident to use a device and its features.  More information on the Digital Inclusion project can be found on the following link
